ok...it is a quest to get this legendary weapon.
first you need this to drop.

then it gives you this http://wow.allakhazam.com/db/item.html?witem=18592
then you combine the Sulfrin hammer *w/e with http://wow.allakhazam.com/item.html?witem=17204 and Wala... you get

WARNING: for Blacksmiths only..but you could make alot of money from those things...lol
